Permission slip not working for me.

The permission slip is not working for me.

Also, can you put a cancellation slip out there in case we need to cancel (at least 2 meetings prior)?

I worked just fine when I tried it a few minutes ago.

I can add a cancel button - but there will be a restocking charge of 25%.
